BOXED WARNING: Fetal Toxicity - "When pregnancy is detected, discontinue captopril tablets as soon as possible," since drugs acting on the renin-angiotensin system "can cause injury and death to the developing fetus." Other key warnings include angioedema of the head, neck, and extremities where "airway obstruction may occur and be fatal," intestinal angioedema, neutropenia/agranulocytosis risk (especially with renal impairment), symptomatic hypotension, and proteinuria/nephrotic syndrome.

Indicated for "treatment of hypertension," alone or with other agents; "treatment of congestive heart failure usually in combination with diuretics and digitalis"; to "improve survival following myocardial infarction" in clinically stable patients with left ventricular dysfunction; and "treatment of diabetic nephropathy (proteinuria greater than 500 mg/day)" in type I insulin-dependent diabetics with retinopathy.
