Uses and warnings

BOXED WARNING: "Special warning on increased risk of cardiovascular mortality" - based on a long-term study of another sulfonylurea (tolbutamide) in which treated patients had cardiovascular mortality about 2.5 times that of diet-treated controls; the label states this warning "is considered to be a class effect" applying to glimepiride and other sulfonylureas, and this risk should be weighed against the benefits of glycemic control. Other warnings/contraindications: diabetic ketoacidosis (treat with insulin, not glimepiride), hypoglycemia risk (greater in elderly, debilitated, or malnourished patients and those with renal/hepatic impairment), and hemolytic anemia in patients with G6PD deficiency.

Glimepiride tablets are "indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus." It may be used alone or in combination with metformin or insulin.
