BOXED WARNING: "WARNING: FETAL TOXICITY" - "Drugs that act directly on the renin-angiotensin system can cause injury and death to the developing fetus"; discontinue ramipril as soon as pregnancy is detected. Other major warnings/contraindications include angioedema (more frequent in Black patients and with concomitant mTOR inhibitors), contraindication in patients with a history of ACE-inhibitor-associated angioedema, with neprilysin inhibitors (within 36 hours of switching), and with aliskiren in diabetics; also hypotension in volume-depleted patients, renal impairment requiring monitoring, and hyperkalemia risk.

Ramipril capsules are indicated for: - Hypertension, to lower blood pressure and reduce the risk of fatal and nonfatal cardiovascular events - Reduction of risk of myocardial infarction, stroke, and death from cardiovascular causes in patients at high risk - Heart failure post-myocardial infarction, in stable patients who show signs of congestive heart failure within a few days after an acute MI.
