Uses and warnings
BOXED WARNING: "Because of reports of intestinal and gastric ulceration and bleeding with extended-release potassium chloride preparations, these drugs should be reserved for those patients who cannot tolerate or refuse to take liquid or effervescent potassium preparations, or for patients in whom these products are not appropriate." Contraindicated in hyperkalemia, which "can produce cardiac arrest," and in conditions predisposing to hyperkalemia; also contraindicated with gastrointestinal obstruction, delayed gastric emptying, or conditions slowing GI transit because of the ulcerogenic potential of concentrated potassium salts. Caution is advised with potassium-sparing diuretics, ACE inhibitors/ARBs, and in renal impairment.
Extended-release potassium chloride tablets are indicated for "treatment of patients with hypokalemia with or without metabolic alkalosis," including digitalis intoxication and hypokalemic familial periodic paralysis, and for "prevention of hypokalemia" in patients at risk, such as those on digitalis who would be endangered by even transient hypokalemia, or those with inadequate dietary potassium intake.
