Active ingredient

Allopurinol

Uses and warnings

No boxed warning. The label warns the drug "SHOULD BE DISCONTINUED AT THE FIRST APPEARANCE OF SKIN RASH," which may progress to more severe hypersensitivity including Stevens-Johnson syndrome, toxic epidermal necrolysis, hepatotoxicity, and death. Patients who develop a severe reaction "should not be restarted on the drug." Dosage of mercaptopurine or azathioprine must be reduced to about one-third to one-fourth the usual dose when combined with allopurinol. Hypersensitivity reactions may be increased in patients with decreased renal function, particularly with thiazide use.

Indicated for management of "the signs and symptoms of primary or secondary gout" (recurrent acute attacks, tophi, joint destruction, uric acid lithiasis, and/or nephropathy), for patients with leukemia, lymphoma and other malignancies with elevated serum/urinary uric acid from cytotoxic therapy, and for patients with recurrent calcium oxalate calculi with elevated urinary uric acid. The label stresses "THIS IS NOT AN INNOCUOUS DRUG. IT IS NOT RECOMMENDED FOR THE TREATMENT OF ASYMPTOMATIC HYPERURICEMIA."
