Uses and warnings

No boxed warning. Contraindications include hyperkalemia, Addison's disease, and concomitant use with eplerenone. Key warnings include hyperkalemia, which can be serious or fatal, requiring potassium monitoring "within the first week" of therapy and periodically thereafter, especially with ACE inhibitors, ARBs, or potassium supplements; risk of hypotension and acute worsening of renal function; electrolyte disturbances such as hyponatremia; and tumorigenicity noted in chronic animal toxicity studies.

Spironolactone is indicated for management of NYHA Class III-IV heart failure with reduced ejection fraction, to increase survival and manage edema, generally combined with other heart-failure therapies; management of hypertension as add-on therapy when other agents are inadequate; management of edema in cirrhosis of the liver when fluid/sodium restriction alone is ineffective; treatment of edema of nephrotic syndrome when other therapies are inadequate; and short-term preoperative treatment or long-term maintenance therapy for primary hyperaldosteronism.
